| An employee was servicing a boiler at a client location. The employee was removing the belt from the motor on the outside of the boiler when the boiler started up, causing the power transmission belt to grab his finger and pull it into the ingoing nip point between the belt and pulley, amputating the tip of his left middle finger. | On October 5, 2017, at approximately 9:00 a.m., three employees were working together to remove vinyl siding from the side walls of a residential balcony for demolition. They were working from extension ladders with hand tools. The injured employee entered the balcony from the extension ladder to look for a potential anchor location for his personal fall arrest system. When he entered the balcony, it collapsed beneath him.
